We do not process insurance at the office, however we do give you the necessary paperwork in order for you to submit to your carrier. This includes a medical necessity letter from our medical provider along with a super bill which provides all the proper ICD-10 Diagnosis codes. You may want to contact your carrier to find out how to submit your claim by using their guidelines. Most carriers have a claim form you need to fill out and have the doctor approve before submitting your claim with the above documentation.
