How it Works
You do it anyway, why not get something for it? Simply print and pass out the Physician’s Plan Referral Card to your family & friends. Remember to write YOUR name and email address on the back. Your friend or family member will receive $20 off their very first visit with us when bringing in the card. You can also pick up printed referral cards at any office locations for multiple use.
The Details
I. For each card returned with YOUR name & email on the back, you will receive 1 point.
II. New patients must spend a minimum of $200.
III. No timeframe. Redeem as you go and anytime.
